What is Bichectomy?

Bichectomy is a surgical procedure that aims to reduce cheek size by removing the Bichat fat pads, located in front of the masseter muscle in the cheeks. This procedure is minimally invasive, with the primary goal of refining facial features, giving the face a slimmer and more defined appearance. The surgery typically lasts about 30 minutes and is performed under local anesthesia. The recommended postoperative rest period is three days, and the results, which are long-lasting, may be visible immediately after the procedure. Recovery is generally quick, taking around 24 hours.

Benefits of Bichectomy

Bichectomy offers multiple aesthetic benefits. Primarily, it redefines the shape of the face, achieving a slimmer and more angular profile by removing excess fat from the cheeks. This can result in a more youthful and attractive appearance. Additionally, it can highlight the cheekbones and improve facial balance. It is a minimally invasive procedure with a quick recovery, usually around three days. The results are permanent and visible almost immediately after the procedure. Since the incisions are made inside the mouth, it does not leave visible scars, thus preserving the natural appearance of the skin.

Bichectomy Procedure

Bichectomy is performed through a small incision inside the cheek to directly access the Bichat fat pads and remove them. Once the incisions are made, the surgeon locates the Bichat fat pads, which are accumulations of fat that we all have in our cheeks. They are then gently removed to avoid damaging the surrounding tissues. It’s important to note that the amount of fat to be removed is decided during the initial consultation, as excessive removal could lead to over-sunken cheeks. It is a short-duration procedure that does not require hospitalization and is done under local anesthesia.

Pre and Postoperative Care

The success of bichectomy requires a detailed preoperative evaluation to detect possible contraindications and appropriate postoperative care. Some of the guidelines for postoperative care include: avoiding tobacco, resting at home for the first three days, avoiding direct sun exposure and physical exertion, and using an antiseptic mouthwash after meals. Postoperative swelling is usually mild and decreases within two or three days, allowing the patient to return to work after the first 24 hours. The final results are typically noticeable once the swelling has completely subsided.

Before and After

During bichectomy, a small incision is made on the inside of the cheek to access the Bichat fat pads and remove them using minimally invasive techniques.

The procedure is performed under local anesthesia, so patients may experience mild discomfort, but not significant pain.

The duration of the procedure can vary, but it typically ranges between 30 and 60 minutes.

Most patients can resume their daily activities within 2 or 3 days, although some discomfort and swelling may persist in the first few weeks.

Initial results are visible immediately after the swelling subsides. The final results are fully appreciated after a few weeks.
